Volunteers’ Undertaking, Declaration and PDPA Consent Clause

In accordance with the Personal Data Protection Act, I hereby give my consent for NeuGen to collect, use and disclose my personal data for any matters relating to my volunteer service.

I allow NeuGen to use my personal data (name, photographs or videos taken of me during different events) for both internal & external publicity purposes.

I accept that I may be subject to a screening process prior to confirmation for the use of my services. This is subjected to as per case-by-case basis.

I agree that NeuGen reserves the right to reject my application, should there be any conflicting reason to do so.

I understand that participating in the programmes/activities of NeuGen involves a certain level of risks. The organisation will not be held liable for any costs, damages, charges and expenses OR injuries, loss and damage incurred during my participation of the programmes/activities.

Consent withdrawal – I may also withdraw my consent to the use and disclosure of my personal data for receiving emails, calls or appeal letters with regards to volunteer opportunities when I do not feel comfortable receiving any more information.
Volunteer Confidentiality Agreement
The agreement applies to all volunteers associated with and/or involved in the activities of NeuGen. All data, materials, knowledge and information generated through, originated from, or having to do with NeuGen or individuals associated with our activities are to be considered privileged and confidential and is not to be disclosed to any third party. Such information can be in these formats but not limited to pages, forms, documents, designs, conversations, messages (received or transmitted), resources, contacts, photographs and is the sole property of NeuGen. I will seek consent in writing with management of NeuGen if there is a need for such information to be retained. I also understand that the consent for the information to be released is subject to the approval of management. In the event of any breach of confidentiality, my volunteering service will be terminated. I may also face legal action in the event NeuGen seeks compensation for any claims, damages or losses suffered as a result of or in connection with such breach. This Volunteer Confidentiality Agreement is deemed valid throughout the duration of my volunteering service. I acknowledge and accept that my agreement herein survives the termination or resignation of my volunteering service with NeuGen.
